Instruction: This form is to be submitted by prospective students who are applying for any of the ICMS programs based on their previous work and/or life experience.
All questions must be answered.

Please note that your application can only be considered if you have submitted all of the required evidence for the criteria outlined in the form.

Your application will then be assessed against you demonstrating a reasonable prospect of success based on:

1 – your previous work, education (formal and non-formal) and life experiences

2 - Your chosen course of study and

3 – The level of the course you have applied for (e.g. Undergraduate or Postrgraduate)

Section 3: Supporting evidence

While providing references, please make sure that they include the time of employment, very clear outline of duties performed, and have a full name and contact number and email address of the referee.
