The following form must be filled out in order to start a new club / group. Once completed, please click the submit button and the form will then be forwarded to the Principal and/or Activities Director for review. Once reviewed, the Principal and / or Activities will contact you with an approval / denial.

Following administrative approval of this application, your club/group must file with the Activities Office: a club constitution (click for constitution example), a membership roster,  and meeting times and location.
 
(The form will be routed to the specific principal / activities or executive director at each secondary school or site upon submission.)
