Pinecrest Pavilion Summer 2019



Class Selections (one form per child)

Class Pricing: Weeks 1, 2 and 4-8 are $195 per class.  Week 3 is $117 per class because of the July 4 and 5 closure.
 
All grade levels listed are based on the child's rising grade level -- what grade s/he will enter in Fall 2019 (i.e., K-2nd means kindergarten through 2nd grade; 3rd-6th means 3rd through 6th grade in 2019-20). 
 
Sibling Discount: Families with multiple children enrolled at camp will pay full camp fees for the child taking the most classes/with the highest balance. For each additional child, a discount of $10 per class is deducted from the total balance. The discount will be reflected in the invoice parents/guardians receive by email in April.
 
Class Availability: If this online form allows you to select and register for a class, your child is confirmed in that class. A message of "Insufficient Quantity Remaining" on this form means a class is full; please select another option. Please email camp@pinecrestschool.org if you would like your child to be waitlisted for a class.

Permissions

I give permission for my child to ride Pinecrest School-contracted transportation for field trips. *
I give permission for my child to ride in personal vehicles driven by Pinecrest staff for field trips. *
I give permission for my child to ride in personal vehicles driven by fellow parents for field trips. *
Photos and videos are taken regularly at school and camp. I understand that with the completion and submission of this summer camp registration form to Pinecrest School that my child’s photo and/or video will be shared with the school or camp community and could be used in school or camp brochures, ads, media coverage, on the school website, on the school's social media sites, et cetera. Names are never used with student photos or videos. Concerns about this should be addressed directly with the Head of School. Additionally, Pinecrest School does not share families’ contact information with third parties. *
● Classes are $195 each, with the exception of Week 3, which is $117 per class (prorated for the July 4 & 5 closure). ● Registration fee is $50 per child (non-refundable, non-transferable). ● Deposit is required at registration – $195 per child (non-refundable, non-transferable). ● Families with multiple children enrolled at camp will pay full camp fees for the chld taking the most classes/with the highest balance. For each additional child, a discount of $10 per clss is deducted from the total balance. *The discount will be reflected in the invoice parents/guardians receive by email in April.* ● A $50 late fee will be assessed for final balance payments and/or required health forms received after the due date. Invoices will be emailed to parents/guardians towards the end of April. The email will come from Pinecrest School quickbooks-email@intuit.com. ● A $25 fee will be charged for returned checks. *
● Provide all required forms (see “2019 Summer Camp Fees, Registration & Forms” on the Pinecrest website) by Friday, May 10, 2019, or within 7 days of registering after this date. If you register later than 7 days prior to the start date, forms must be submitted by the first day of class. ● Children without proper paperwork will not be allowed to attend until all forms are submitted, and a $50 late fee will be assessed. *
● If you want to change, add or drop a class, email camp@pinecrestschool.org directly. ● After Friday, March 15, 2019, no drops or changes will be allowed, no refunds will be given, and parents/guardians are responsible for paying in full for all classes for which your child is registered. ● All changes are subject to class availability. ● The deposit and registration fee paid at the time of registration are non-refundable and non-transferrable. ● Refunds are not given for absences or partially-attended sessions. ● There will be a $10 per class fee for any class changes after registering but before Friday, March 15, 2019. *
