Post Hospital Discharge Services:
For individuals who are currently hospitalized for psychiatric and/or substance use disorder care and require follow-up mental health services within 7 days of their expected discharge.
Outpatient Services:
Therapy and/or psychiatric medication management to address mental health concerns such as depression, anxiety, trauma, etc.
Other/Unsure:
The service you are seeking is not listed or you're unsure what service is needed and desire an assessment.
Assertive Community Treatment Team (ACTT - Adults):
A community-based team who support an individual diagnosed with severe and persistent mental illness with long-term psychiatric disability. Typically, this individual has had frequent hospitalizations for psychiatric needs. Psychiatric medication management is included in ACTT services.
Community Support Team (CST - Adults):
A community-based team who support individuals with a mental health or substance use diagnosis with significant impairments in other areas of their life as a result of their diagnosis.
Peer Support Services (Adults):
Support for people with a mental illness or substance use disorder provided in the community by a certified peer support specialist with lived experience. May include assistance with daily living skills, social skills, utilizing community resources, and maintaining overall health and wellness.
IPS-Supported Employment (16 years and older):
Assistance in choosing, acquiring and maintaining employment for individuals diagnosed with severe and persistent mental illness.
Psychosocial Rehabilitation (PSR - Adults):
A day program for adults whose severe and persistent mental illness inhibits their independence in the community, and who require support with job skills, managing finances, social skills, and utilizing community resources.
Intensive In-Home Services (IIHS - Ages 3-20):
In-home and community-based support for children/adolescents (and their families) who have a mental health diagnosis and require more intense treatment than outpatient services.
Peer Bridger Services (Adults):
Peer Bridger services provide short-term support for individuals transitioning out of the hospital and requiring connection to community services. Only available at designated hospitals.
