I understand the inherent risks involved in my son/daughter participating in soccer. By signing this form, I waive the liability of Star City Soccer, Crossbar Training, and Valley AFC for any injuries sustained by my son/daughter while participating in Winter Training and Indoor League. I also understand that the Star City Soccer, Crossbar Training, and Valley AFC staff will initiate any appropriate medical attention that my child may require to the best of their legal ability.
