Youth Spirit Award Application
for Members in Grades 9 thru 11

The New York State USBC Youth Spirit Awards will be presented to up to three youth bowlers who are nominated by their youth bowling coach who meets the following criteria:

• New York State USBC Association youth member in good standing.
• Must be currently enrolled in grades 9 to 11.

The winner(s) of this award will be selected by the New York State USBC Scholarship Committee based on the applications received.

Coaches - We suggest you use the following criteria when nominating your youth bowler for this award:

Sportsmanship
Commitment
Leadership

This award will not be presented solely based on the nominees’ bowling achievements!

Please write a statement on why you feel your nominee should be the recipient of the New York State USBC Youth Spirit Award. Please refrain from using the nominees name in the statement.
