PARTICIPANT WAIVER AND RELEASE FOR MINORS

I understand and acknowledge that mentioned activity, generally very safe, may poses risks to my child. I (we), as parent(s) or guardian(s) of the minor, do hereby, for my child, myself, my heirs, executors and administrators, release and forever discharge Tacoma Music Academy and all officers, directors, employees, agents and volunteers of the organization, acting officially or otherwise, from any and all claims, demands, actions or causes of action which in any way arise from the minor’s participation in the above noted event. I do hereby certify that to the best of my knowledge and belief said minor is in good health. In case of illness or accident, permission is granted for emergency treatment to be administered. Policies & Procedures

1. TUITION. All tuition payments are processed monthly by automatic charge to credit or debit cards. Tuition is never prorated for missed lessons. Tuition does not include any retail teaching materials such as books or instruments. Tuition may be subject to revision based on CPI.

Accounts with past-due balances may result in the suspention or cancelation of scheduled lessons until the balance is paid in full or an arrangements are maid to pay it in full. *

2. CANCELLATIONS AND MAKEUPS. For any private lessons missed by a teacher, the Academy will attempt to provide a substitute teacher. If a substitute teacher is unavailable, a private makeup lesson with the teacher will be scheduled promptly. For any lesson missed by a student, the student can attend a group make up lesson. To sign up for a makeup lesson, contact the office or your instructor for a date and time. We do not offer make ups for missed group lessons. *

3. DISCONTINUING LESSONS. To discontinue lessons and to discontinue charges to your account, fill out the withdrawal form before the 15th of your last month.

We do not prorate tuition for the last month’s lessons. Students will be responsible for their last month’s tuition whether or not they attend lessons. *

4. AVERAGE OF FOUR LESSONS PER MONTH. Monthly tuition is based on a 48 week year. We are closed four weeks for holidays. In the long months, you will get five lessons, short months such as November and December, you will get three lessons, and most months you will get four lessons. The tuition is the SAME each month; i.e. we don’t charge you more when you have five lessons, and we don’t charge you less when you have three lessons in a month. This makes everyone’s accounting easier. *

5. SUMMER VACATION. The TMA operates year round and does not close for the summer. We are happy to offer unlimited group make up lessons for our students, and they are held every week. If you are taking a month off for vacation and want to resume the next month, your payment is still due on the first of the month to reserve your lesson space. The lessons for the month you miss can be made up when you return from vacation. *

6. CARE OF STUDENTS. The academy is not responsible for providing before or after care for students. Parents with children under the age of eight must remain in the school lobby during the classes. Students are not to be left at the school for excessive time periods before or after their lessons. Children must wait for their parents to pick them up inside the Academy. Please do not ask them to meet you elsewhere. The academy assumes no responsibility for children who leave unaccompanied. *

7. OPEN DOOR POLICY. Parents are always allowed to sit in on lessons. We have an open door policy for lessons. During lessons parents should not comment or otherwise interfere with the instruction. *

8. INJURIES AND PROPERTY DAMAGE. Parents, legal guardians of minors, students and adult students waive the right to any legal action for any injury sustained on school property resulting from normal lesson activity or any other activity conducted by the students or their siblings before, during, or after lesson time, including hearing loss or hearing damage. Tacoma Music Academy is not responsible for any damage done to vehicles or items stolen from vehicles. *

9. LIABILITY. Parent or legal guardian is responsible for any damage to the instruments or other equipment caused by the Student. Parent/Student if adult is expected to reimburse the cost of repair or replacement. *
