Medical Information
In the event of a medical emergency and the above listed emergency contacts cannot be reached and immediate medical treatment is necessary, please read and consent to the below terms:
MEDICAL TREATMENT/TRANSPORTATION: I hereby grant permission to Cascade Christian Schools to seek medical treatment for myself in the event such treatment is deemed necessary, to include being transported by an emergency vehicle to a medical facility for treatment.
ADMISSION AND/OR PHYSICIAN’S CARE: I hereby consent to all medical and surgical treatment by the attending physician and to any necessary examinations, administering of medication, anesthesia, x-rays, blood tests, transfusions, suturing or other medically necessary procedures for myself while I am hospitalized.
FINANCIAL AGREEMENT FOR MEDICAL TREATMENT: I agree to accept responsibility for any financial indebtedness incurred during the hospitalization. I agree to pay for all necessary medical services at the current rate and in case of collection, pay a reasonable attorney fee and collection expense.
