MEDICAL TREATMENT/TRANSPORTATION
I hereby grant permission to Cascade Christian Schools to seek medical treatment for my child, in the event such treatment is deemed necessary and, for my child to be transported by an emergency vehicle to a medical facility for treatment.

HOSPITAL ADMISSION AND/OR PHYSICIAN’S CARE
I hereby consent to all medical and surgical treatment by the attending physician and to the administration and performance of all examinations, administering of medicine, treatments, anesthetics, operations, x-rays, blood tests, transfusions, suturing or other procedures, which may be deemed necessary for my child, during the stay at this hospital.

FINANCIAL AGREEMENT FOR MEDICAL TREATMENT
I hereby agree to accept responsibility for any financial indebtedness incurred during the hospitalization. I agree to pay for all necessary services at the current rate and in case of collection, pay a reasonable attorney fee and collection expense.

FINANCIAL AGREEMENT

Cancellations: Cascade Christian Schools reserves the right to cancel any class not meeting minimum enrollment. In the event of a cancellation, you may select another class or request a refund. However, the refund will first be applied to any charges on your FACTS account. Refunds may take up to 3-4 weeks to process.

Withdrawal:Cascade Christian Schools has the right to enforce withdrawal of any student whose conduct is deemed undesirable or unsatisfactory. Failure to conform to CCS standards of behavior may result in immediate dismissal without refund.

Cancellations/Refund Policy: Due to limited space available, and the advance purchases of supplies/equipment, only cancellations received 72 hours prior to the start of the activity will receive a partial credit to your account. If there is an outstanding balance owed to CCS, the credit will be applied to the outstanding balance.

If there is a schedule change that CCS needs to make to enrichment classes and/or athletics because of an unavoidable situation, we will offer a combination of make-up sessions and/or refunds, depending on the specifics of the situation. This will be determined at the time by CCS.

Payments are processed via PayPal. You do not need a PayPal account to process your payment. PayPal will allow you to pay as a guest, using electronic card payment, or e-check.
