Educational Opportunity Application

Please complete the following information. This form will be utilized for all applications throughout the year for AVAHO education.

NOTE: AVAHO has the right to review all educational materials prior to giving final approval, e.g., slide decks. Educational programs that are relevant to all AVAHO members are preferred. AVAHO members include physicians, nurses, pharmacists, nutritionists, cancer data analysts, social workers and more.

So that the Education Committee can objectively evaluate the quality of each submission, please complete the following as thoroughly as possible:

Selection Criteria:

All applicants will have equal opportunity to submit applications for educational programs. Applications will be evaluated based on the following criteria:

The program description adequately describes the content to be presented;
The learning objectives are clear, objective, and well-developed;
Based on the description and objectives provided, the program appears relevant for a significant portion of the AVAHO membership (e.g., beyond one or two disciplines such as physicians and pharmacists);
The proposed topic is particularly relevant to the veteran population/VHA;
The presenter is a known expert in the field (bio/CV submitted), or a VA professional has been requested;
Program content has not been presented to the membership in the past year.

Please note: A new drug for a disease that has been frequently covered (or for a new indication) is less favorable.

Available Program Types (Please Choose One): *

Educational Program during the AVAHO Annual Meeting ($50,000): Held during breakfast or lunch (must be CE-eligible and meet non-branding requirements as set forth by CE provider before acceptance)

On-Demand Webinar ($5,000 non-CE): Webinars stored on the AVAHO member website for a predetermined time (usually 12 months unless otherwise negotiated). AVAHO will market the opportunity to its members and provide viewership analytics to sponsor. Live stream sessions may be placed on-demand following live session by request. While CE-eligible webinars are encouraged, AVAHO will consider non-CE topics of high interest to its members. Please contact AVAHO for CE-eligible training options and costs.

Live Stream Session/Webinar ($5,000 non-CE): Live, virtual sessions with accompanying registration and active participation by attendees. AVAHO will assist in marketing to its members. Please contact AVAHO for CE-eligible training options and costs.

Other: AVAHO will review special requests for educational programs, opportunities, and information that is beneficial to its members. This may include a series of events or workshops, regional meetings, special event, round table discussion, or other innovative content delivery.
