I, the minor's parent and/or legal guardian hereby give responsibility for emergency medical decisions to a responsible adult of the Red River Valley Figure Skating Club in my absence, and will not hold them responsible in any manner when such decisions are made in conjunction with appropriate medical person/s or institution. I also release appropriate medical personnel and/or medical institutions from any responsibility concerning medical care and will be financially responsible for such appropriate care as deemed necessary. Sanford Hospital and/or Sanford Medical Group (doctor on call) will be the facilities used for medical attention.

Upon registering with the Red River Valley Figure Skating Club, I agree to hold and save harmless the Red River Valley Figure Skating Club, U.S. Figure Skating, the City of Moorhead, its directors, officers, administrators, sponsors, volunteers, agents, employees, staff, and instructors jointly and severally, from any claims for injuries sustained. I agree to provide insurance coverage for skater/s and understand the Red River Valley Figure Skating Club carries no coverage for personal injury.

I understand that this activity involves risk of serious bodily injury which may be caused by my skaters actions or inactions, those of others participating, the conditions in which the activity takes place or the negligence of the "releasees" named above. I represent that my skater is in good health and in proper physical condition to participate in ice skating. I acknowledge that if I believe event conditions are unsafe, I will immediately discontinue participation.

I have read this release and waiver of liability, assumption of risk and indemnity agreement, understand that I have given up substantial rights by signing it and have signed it freely.
