Habitat for Humanity of Huron Valley Furnace Programs

Are you a homeowner living in an owner-occupied, detached, single-family home in Washtenaw County? The furnace to be tested must be located in this same home. BOILERS ARE NOT ELIGIBLE. Rental units are not eligible. Multiple-residence structures including condominiums are not eligible. *
Does DTE Energy provide the electrical and gas service to your home? *
Is your annual household income less than than the maximum amount listed below for your household size or do you participate in one of the following Michigan, federal, or nonprofit low-income programs: LIHEAP, FAP, SSI, SER, AFDC, TANF, WAP, THAW or WIC?

Have you had a furnace inspection/clean & check through a DTE sponsored program in the previous five years (sixty months)? *
I agree to allow Habitat for Humanity of Huron Valley to arrange for an approved HVAC contractor to schedule a tune-up of my heating system. I agree to the installation of the following energy savings measures* in my home: up to 25 LED light bulbs, low-flow kitchen and bathroom aerators, and a low-flow bathroom showerhead.

I understand that should the HVAC contractor determine that my furnace is unsafe or un-repairable, Habitat may (at its own discretion) provide the option to replace my furnace. Should this be the case, at that time I understand that Habitat will discuss with me the requirements for the furnace replacement. Those requirements may include, but are not limited to, providing additional income and household documentation, agreeing to payments to Habitat for Humanity of Huron Valley in the amount of $40 per month (for furnace replacement only) or $60 per month (for furnace and water heater replacement), at 0% interest for four years, and agreeing to complete eight volunteer hours in approved Habitat for Humanity activities. If I am unable or unwilling to provide the additional required documentation, Habitat, at its discretion, may replace the furnace and/or water heater, and I will be responsible for repaying the full replacement cost instead of the discounted price. *
I verify that I am current on any/all payments to Habitat for Humanity of Huron Valley. *
