I understand that cheerleading and dance related activities present a risk to the participant. I understand that there is an inherent risk of injury that cannot be eliminated regardless of the care taken to avoid injury. I agree to assume that risk in order to participate in these activities.

The undersigned acknowledges that the participant is in good health and does not have any history of medical or physical condition that would place the participant at risk due to their medical or physical condition.

I also agree to pay all necessary fees with East Coast Allstars on time or I will be redirected to a debt collector's agency

I understand that payments will be made by direct debit on a Term basis and I agree to continue paying including exit fees after I give East Coast Allstars written notice that myself/ my child will no longer be participating.

I agree to all Tier team fees and will still pay all assiocted fees regardless of leaving the program.

I do hereby release and forever discharge East Coast Allstars, their predecessors and successors, employees, agents and assigns, and all other persons, corporations, and entities from all claims, expenses, attorney fees, and causes of action or suits of any kind or nature associated with the participant’s involvement with East Coast Allstars.

I understand the East Coast Allstar class fee and registration fee are all non refundable and I agree to pay the required exit fee should I choose to leave the program.
